  • Our plan includes a 30-day money back guarantee. You can cancel and receive a full refund up to 30 days after purchase if no benefits have been used. Refunds and cancellations after 30 days will be our discretion.
  • Payment for treatment(s) not included in the plan is due at the time of service.
  • Plan discounts do not apply to the following procedures completed at our practice:
    • Cosmetic treatments such as cosmetic bonding and veneers.
    • Implant Services (CDT Codes D6000-D6199)
    • Orthodontics such as comprehensive orthodontic treatment and appliance therapy (CDT Code D8000-D8999)
    • Adjunctive General Services such as anesthesia (CDT Codes (D9000-D9999)
